Flyers and FormsWhen published in printed form the Newsletters have the following attachments. For performance reasons these Flyers and Forms are displayed separately from the 4th Newsletter onwards. Flyer The Flyer gives the group's contact details and can be displayed in any suitable location where people may be looking for information about birthmarks .e.g Maternity wards, GP's Surgeries etc. Gift Aid This form enables the group to recover income tax on any donations made by Income Tax payers. Simply complete the form and send to the group's PO Box listed on the Contacts page. Membership This is the offline membership form for members that don't have internet access. Online UK residents can join via the Contacts page. Clothing This is the Sweatshirt/T-shirt order form for Adults and Children's clothing all carrying the Birthmark Support Group Logo. Merchandise This is the Merchandise order form. Items include Shopping Bags, Lanyards, Car Air Fresheners and Shopping Trolley key rings.
